If I ask, you will inform me if a consumer report was requested and the name and address of the consumer reporting agency that furnished the report. You are authorized to check my credit, verify my identity, verify my employment history and answer questions about your credit experience with me. I hereby authorize the release of any and all requested information to American Finance, L.L.C. for the purpose of initial processing and future continuing maintenance of my sales contract and application. Requested information may include, but is not limited to, verification of savings/depository account balances, employment status, position, shift, department, employment history, and earnings records. I understand that this information will be kept in strict confidence. I have read and acknowledge receipt of the Privacy Notice. If I provide you with a cell phone and/or email address now or in the future, I am authorizing you to contact me via phone call, text and/or email. An original or copy of this form shall be considered valid authorization for American Finance, L.L.C. to request and any of my previous, present or future creditors to release the above-referenced information.
